The Attorney General is considering what action should be taken on a report on the death of Ciara Dunne, originally from Burt, and her family in County Wexford last year.
The report was presented to the Minister for Youth and Children, Barry Andrews TD, on Tuesday and he sent it to the Attorney General for a legal opinion, a spokesperson at his department told the Journal.
The report was delayed on a number of occa
sions while legal issues were addressed.
The bodies of Adrian Dunne, his wife Ciara (née OBrien) and their two children, Leanne and Shania, aged five and three, were found at their house in Mongeer in County Wexford in April 2007. Three days previously, the couple had met a local undertaker to discuss funeral arrangements for the family.
It later emerged that the authorities had been made aware of a possible risk to the safety of the children.
An independent inquiry into the tragedy was set up to establish the circumstances surrounding the deaths and how state services dealt with the family before the tragedy. It was chaired by Barrister Kate Brosnan and was closed to the public.
The report, due to be published in the coming weeks, is expected to recommend the introduction of an out-of-hours emergency social work service to help deal with crisis situations involving children.
