“The approach is in line with the aims and objectives of the NHS policy ‘Making Life Better’ which seeks to make services more accessible for people in the community by supporting and empowering them to make the best life choices with the support of local health professionals. The aim is to create the right environment for people to take greater control over their lives and be enabled and supported to improve their health andwell-being by providing better access for people who have concerns about their physical or mental health or who are suffering longterm or age related conditions.”
